An acute myocardial infarction occurs when there is a blockage of blood flow to the heart muscle, this is called ischemia which means lack of oxygen rich blood to the myocardium. This lack of blood flow leads to signs and symptoms of chest pain that is either sharp and stabbing pain, or commonly described as heavy pressure over the left substernal. Immediate pharmacological interventions of MONA drugs are typically administered, but given in the order of: Oxygen, Nitroglycerin, Aspirin, & Morphine is usually given last. Diagnostic tools include an EKG where nurses look for ST elevation (STEMI), and blood labs of troponin, myoglobin, CK and CK-MB levels are drawn, but according to the NCLEX, troponin levels are the priority cardiac enzyme lab indicating an MI. Complications after a myocardial infarction include: pericarditis, cardiogenic shock, ECG EKG dysrhythmias like V tach & V fib, as well as heart failure in most patients..
